New evaluations require 50% of full payment at the time of scheduling an appointment.
Full payment is due 2 BUSINESS DAYS (48 HRS) prior to the appointment.

SAFETY MEASURES
Although masks are optional if vaccinated, please consider wearing a mask or remaining socially distanced when visiting our office. Please consider a remote session or rescheduling if feeling ill or experiencing respiratory symptoms of coughing or sneezing.
FIRST APPOINTMENT
For in-person sessions, please bring in paperwork or arrive 15 minutes prior to the first appointment in order to complete new patient paperwork.

APPOINTMENT DURATIONS
- New Patient Evaluations: 75 minutes
- Follow-Up Sessions: 25 or 50 minutes
- Consultations: 25 minutes or 50 minutes
Dr. Goble will provide an initial diagnosis, discuss treatment options and recommend follow-up appointments as clinically indicated.

Payment
Dr. Goble is an out-of-network provider and does not contract with any insurance carriers. Fees for service are due at the time of appointment and may be paid by cash, debit or credit card. Patients seeking to submit a claim with their insurance carrier for out-of-network reimbursement may request the necessary documentation to support the claim.
